Determinant of childhood stunting in Indonesia: A path analysis using population-based data

Abstract


Stunting is a major public health problem in Indonesia. It has several negative health consequences, include increased mortality risk in children, decreased cognitive development, and increased the risk of chronic diseases during adulthood. Although the government has implemented various policies and interventions to reduce stunting; however, the prevalence of stunting is still high. From 2012 to 2020, the stunting reduction in Indonesia was approximately 3.7 percentage points from 34.5% to 30.8%. This reduction was slightly slower than in Southeast Asia countries, by about 3.6 percentage points. The UNICEF Framework for child undernutrition classified three essential components as the underlying causes of stunting: (1) socioeconomic status, (2) feeding practices, and (3) infection. The poverty inequalities between provinces tend to influence variations in the prevalence of stunting in Indonesia. In addition, cultural differences between ethnic groups also affect feeding practices and immunization coverage in children. This study aims to investigate theoretical pathways to stunting among children under five years of age, using population-based publication data that include the 2013-2021 National Socioeconomic Survey, 2013-2018 National Basic Health Research, and the 2021 Indonesian Nutritional Status Survey. We use path analysis to examine two hypotheses: (1) socioeconomic status inequalities, feeding practices, and basic immunization status are directly associated with stunting; and (2) socioeconomic status and basic immunization status are associated with stunting through feeding practices. The findings of this study are expected to provide a better understanding of stunting determinants between provinces in Indonesia. They can be used as a basis for designing policies to accelerate stunting reduction.
