Analysis of Drug Related Problem Inpatient with Ischemic Stroke in Koja District Hospital February - April 2019

Abstract


Background: Providing drug therapy for ischemic stroke patients requires special attention and ischemic stroke patients often experience complications with other degenerative diseases so that they usually have a tendency to use drugs of more than five types or called polypharmacy. Objective: Analyze drug related problems with the domain of therapeutic effectiveness and drug reactions that are not desired under the Pharmaceutical Care Network Europe. Materials and Method: The method of this study is cross sectional using medical record data, prescriptions, and nurse records. Result: Samples from this study were patients with a primary diagnosis of ischemic stroke and patients aged ≥23 years. Result Analysis was carried out on 115 study samples. The most common drug related problem is the problem of therapeutic effectiveness of 65.00% with the highest sub domain, there are drug effect is not optimal of 29.58%. Other drug related problems is the problem of unwanted drug reactions has a percentage of 35.00% with the highest sub domain is the patient suffering from an unwanted drug reaction instead of an allergy of 34.58%. The highest cause of the problem identified in this study was that the combination of drugs or food-drugs was incorrect including the incidence of drug interactions, which was 56.04%.
