4. Description Abstract

Warfarin is an anticoagulant with a narrow therapeutic index. Not reaching the target of International Normalized Ratio (INR) in patients taking warfarin caused complications. If INR lower then target could risk for thromboembolic event, otherwise if above the target could risk for bleeding. The purpose of the study is to identify the factors that influence the achievement of target INR in outpatients who taking warfarin therapy. This study uses cross sectional design with non-probability sampling at 90 outpatient who taking warfarin at RS Jantung dan Pembuluh Darah Harapan Kita. Data analysis using T-test, Chi Square, multiple logistic regretion. The results showed probability of achievement INR target in adherence group increased 3,8 times than nonadherent group after controlled by comorbidity. Level of knowledge about warfarin in outpatients RS Jantung dan Pembuluh Darah Harapan Kita are low at 74,4%. It’s necessary to provide education-related and motivation to improve patient compliance to medication and maintain of comorbidity control in order to secure the use of warfarin
